Patient Records Requests

To request patient records, please contact us via one of the following:

Requests must provide sufficient detail to establish that the requestor is authorized to receive the records.  

  • Patients may request copies of their own chart by submitting a written request to our records request department. Using this form will ensure our staff has all the information needed to promptly process the request: Patient-Provider Access Request Form.pdf 

  • Healthcare providers who need medical records for the current treatment of a mutual patient may submit a written request to our records department using the Patient-Provider Access Request Form.pdf or their own written request. 

  • Third parties, including patient representatives and law firms, may use this HIPAA-Authorization-for-Release-of-Records.pdf or a similar HIPPA-compliant document that meets the requirements of their state. Where relevant, a subpoena may also be issued and sent to us at Healing Partners, Attn: Records Request, 909 Ridgebrook Road, Suite 300, Sparks, MD 21152 

Compliance Issue Reporting

Employees, patients, and customers of Healing Partners can report questions or concerns regarding confidentiality; potential fraud, waste and/or abuse; or mismanagement related to HP’s policies and practices by using any of the following methods:

HP’s Compliance Officer and members of HP’s Compliance Committee will receive information provided and will investigate/address the issue(s) raised with appropriate members of management. The information provided will be kept confidential, to the extent possible, in accordance with HP’s compliance policy/procedures and subject to government laws.

HP’s Compliance policy prohibits retaliation against individuals who report activities that may violate laws, regulations, or Company policies. This includes former as well as current officers, employees, and contractors. Each report is taken seriously and thoroughly reviewed to make sure proper action is taken.
